Acknowledgement, Waiver and Release

I, {name}, hereby acknowledge that I am aware and understand that the art and sport of karate is potentially hazardous and that a participant is exposed to risks inherent in said activity. I consent to receiving First Aid treatment should I become injured.

I further acknowledge that in the course of doing karate I may receive injuries that may be serious and permanent.

I hereby acknowledge and accept all hazards and risks involved in the said activity and hereby remise, release and forever discharge the Edmonton Gojukai Karate Club and the Edmonton Japanese Community Association, their directors, officers, instructors, servants, employees, successors, agents, volunteers, and assigns from any and all claims and demands whatsoever for damages, loss or injury, however arising which may now or may hereafter be sustained by me in consequence of my participation in the karate program including, without limiting the generality of the foregoing, training, practice, competition, demonstration, travelling to or from any of the foregoing, and any other matter related in any way to the said activity.

As a participant of this karate program, I recognize and accept that there is a risk of contracting COVID-19 due to my participation. The organizers have done their best to minimize risks, but I also acknowledge my role. I will follow provincial and municipal guidance and bylaws while in attendance, including not attending if I have COVID-19 symptoms and/or have been in contact with someone who tested COVID-19 positive.

Under the Occupiers’ Liability Act, I recognize I cannot hold the Argyll Community League, the Edmonton Japanese Community Association, or the Edmonton Gojukai Karate Club liable for a COVID-19 outbreak or transmission caused by negligence on my or my next of kin’s behalf.
